Setting up your first appointment is super easy – just call us at 510-462-3771 or request an appointment online. To get you into our system and onto our schedule as quickly as possible, please have the following information available when you call us:
(1) your prescription or referral (if you have one),
(2) your insurance card (if you intend to use insurance to pay for a portion of your treatments), and
(3) your calendar.
Please bring :
**It generally takes about 10 minutes to complete our intake packet. Although we’re happy to provide you with a packet at any of our locations, you can save time on the day of your evaluation by downloading, printing, and filling your paperwork ahead of time.
Please wear or bring comfortable workout clothes (including shoes and socks). If you use an assistive device (like a cane or walker) or use other braces or appliances (like shoe orthotics) that relate to the problem you need help with, please bring that with you as well.
Please arrive 10-15 minutes prior to your evaluation to allow for parking and in order to complete the appropriate paper work (if you have not already downloaded our forms above). At that time, our office staff will review your insurance benefits if your insurance information is available.
You will then be evaluated by one of our licensed physical therapists. Your evaluation will be a one-on-one session in which the clinician will perform a thorough exam to determine the cause of your problem or complaint. Based on this evaluation, you will be consulted on your diagnosis and any questions you have about your individual therapy plan will be answered. You will also be issued a home exercise program that is prescribed by your clinician for you. Frequently there is also time for a short treatment, during which immediate relief of your problem area or pain can be achieved.
Your treatments will be overseen by your Physical Therapist, with assistance from our Transitional Trainers and Administrative staff.
After a comprehensive evaluation, goal assessment, and treatment plan, you will be scheduled for return visits based on your Physician and Physical Therapist’s recommendation.
Our most important goal is to help you get fast relief or answers to your problem, and to meet your long-term rehabilitation and treatment goals and prevent further injury.
